What Causes Headache Along With Large Bump On Temple Area?

Question: My grandson is at the third Doctor this morning

He has been having head aches since the first of January. This morning a bump about the size of a grape appeared on his temple. Within a half hour it disappeared. However earlier while getting ready for school his dad noticed his eye was read and he asked XXXXXXX if someone had hit him as it looked like he might be getting a black eye.
The first two Doctors earlier this past month said they couldn't tell anything was wrong and referred him to a pediatrician. Today his parents took him to a pediatrician in a larger town. They are waiting for an appointment for an MR which they were told wouldn't put off as much radiation as an MRI. XXXXXXX is 10 and was born with a massive light red tiny vein type birthmark that covered his hand and his arm. He had two laser treatments on the birthmarks in San XXXXXXX when he was about three and four that decreased their visibility considerably.
The doctor today said he wanted to be sure wasn't some tangled vein mass in his brain related to the birth marks. It took several years for a correct diagnosis for the type of birthmark he had.

It is really sad to know all this. From your clinical history, I would like to rule out Sturge-Weber syndrome. In this condition there is some vascular abnormality present with congenital birth mark called Port Wine stain - which is also a vascular abnormality. The condition can also involve eye structures leading to glaucoma which can cause headache and reddening of eyes.

I would suggest you to go for MR. In this there is same principle as MRI but it is more appropriate in diagnosing vascular malformation.

Also consult an ophthalmologist for the diagnosis of glaucoma if it is there and its management accordingly.

Dear Dr. XXXXXXX

Thank you very much for your quick and thorough reply. I apologize for not responding sooner. We live in Alaska and Alaska time is one hour later that Pacific Time.

The pediatrician XXXXXXX saw yesterday said very much the same thing as you did. He didn't mention Sturge-Webster Syndrome but did discuss his birthmark. He scheduled XXXXXXX for an MR next week. The Doctor didn't prescribe anything for the headaches just yet because he needs to be sure what they are dealing with first.
XXXXXXX has never displayed any signs of retardation. He is extremely bright and always is either first or second in all of his classes. He loves school and is excellent in sports. He has a nearly photographic memory, as does is dad, but has always been a bit of sickly, allergies, asthma, sick and/or hurting stomach and headaches and those sorts of things.

The only visible malformation is his left arm and hand have that lighter veinish birthmark all the way up I think to his neck and also that arm and hand are slightly larger than the normal right hand and arm. Him mother noticed it immediately when he was born.

It's good to know that he is sharp and intelligent. Most of the children with sturge Weber syndrome have some kind of intellectual abnormality and seizures. Even the skin lesion you have described is not characteristic of this syndrome. Normal intelligence in your grandson suggests that his brain has developed well which is not usual if there is vascular malformation in the brain as there are higher chances of poor growth of brain. He might not have this particular syndrome but any other condition similar to this.
MR will show if any abnormality is there.
Prognosis of the disease also depends on MR findings.
Did you visit the ophthalmologist for his eye check up ? I am worried as you did not mention his eye's condition.If you have not visited, I would suggest you to get an urgent opinion as it may be emergency condition.
