What Causes Headache, Decreased Appetite, Severe Epigastric Pain After Eating And Hay Fever?

Question: Hello Dr Rynne, thank you for your time today. I have been unwell for a long time and have been having tests done which do not seem to provide any real answers. I am 55 yrs of age of slim build with 4 adult children. I have all the syptoms associated with stomach ulcers but the h. Plori bacteria test proved negative. I had thought that I may have developed some food intolerances so have been eliminating white bread, rye and wheat from my diet. I have not been too vigilant though as I am concerned that I will not receive enough vitamins and minerals. I suffer with headache/ migraine, lack of appetite anxiety extreme discomfort after eating some foods, hay fever, gas etc . Each pregnancy resulted in severe morning sickness and hospitalised I have also had a parasite grow in my bowel and was hospitalised for that. I was diagnosed with Hepatitas B when I was 21 resulting in 3 months off work but fully recovered. Any suggestions would be most appreciated. Thanking you , kind regards XXXX.

I read your question and i understand your concern. Age of 55 with 4 kids and symptoms of gastric discomfort, bloating, post prandial worsening of GI symptoms, negative for H pylori, history of headaches, migraines anxiety and stress and distension and allergies like hay fever point towards a functional bowel decline and irritable bowel syndrome is on top. Hyperemesis gravidarum in every pregnancy more than usual indicates IBS as well, although emesis means a healthy baby as body gets very skeptical of any kind of food and only allow some of it to make sure nothing hurts the baby during organogenesis phase of development.

Now the symptoms, Usually peptic ulcers are epigastric pain, worsen after meals, and has history of NSAIDs and some of your features may favour that but with negative H pylori and no NSAIDs history , its highly unlikely but still surveillance endoscopy should be done along with one month Proton pump inhibitor trials should be considered.

Modifying diet and lifestyle is mainstay of treatment here. Small frequent meals , Avoiding trigger foods like dairy products, fatty foods, spices, processed canned foods, cheese products, coffee, alcohol, beverages, tea, coffee , etc. Use more fibers, fluids and prefer fresh fruits and vegetables. Exercise daily, reduce weight to less than BMI of 25 and delay lying down after meals for at least 3 hours . Keep head side of the bed elevated at least 8 inches. If the issue persists consults a gastroenterologist for further guidance, you will get plenty of literature and supportive home remedies for some authentic sources for patient education like ebmd, mayoclinic etc.
