What Causes Heart Palpitations?
Question: Diabetic for 6 months. I was feeling heart palpitations for the last one week. So I went to see a cardiologist who adviced me an ECG and TMT. ECG showed SINUS BRADYCARDIA while TMT showed "Evidence for positive provocable myocardial ischemia". The maximum dip was in v6 for 0.07mV The blood reports including Lipid/Cholesterol etc were all normal. I do not have shortness of breath but take deep breath at say 5-6 minutes. I do not feel any acute chest pain (a bit chest pain occurs probably due to strain in muscles from exercise). No other symptoms. On showing the report to cardiologist, he said the TMT report is incorrect and said I was normal. The palpitations (heart appears to be pounding hard) were normal and psychological in nature. I am confused and worried, please help.

I would go with the opinion of your Cardiologist and would reassure you that there is no cause for concern.
Isolated single lead abnormality especially the outermost lead V6cannot be considered significant. Also you do not seem to have any other symtoms attributable to Ischemic heart disease.

I have another query: The depression was not only in V6, but also in V5. The report said : XXXXXXX depression in V6(-0.07 mV) at Stage 3 of Exercise. Is this significant and worrisome? Also my XXXXXXX HR was 153 which was 80% of XXXXXXX predicted(191 bpm). Is this normal?

Any depression of more than 0.1 mv is significant.
There is absolutely no reason for being worried.
Also any hear rate which is reached above 75 percent of predicted heart rate is considered acceptable. It need not be 100 percent of predicted as there can be variations among all individuals.
I would like to reassure you that I would back up the decision of your Cardiologist.
For your reassurance you can repeat the test after 6 months.

Hello doctor! Thanks for your reply. I have one more query: As I had mentioned earlier, the ST depression in v6 was maximum of -0.07mV. And as you have said, any depression of 0.1 mV is significant. So, would it be safe for me to assume that the ST depression in my case is not very significant(since 0.07 < 0.1) and I am fine? Also lately i have noticed that i breathe heavily when i climb a flight of stairs (but not so when walking briskly) . Please help. Thanks again!

Yes there is no reason for concern about ST depression. Climbing stairs requires more energy as its against gravity and also due to prevailing weather and anxiety(cardiac anxiety) could cause heavy breathing. There is absolutely no cause for concern.
Meditation and daily brisk walking for 30minutes at a stretch will b helpful.
