Hello ma'am,
Sorry for such a long delay in my response.
I have gone through your history and in my opinion at 45 yrs.of age and high fsh level there is a chances of postmenopausal or perimenopausal stage.
But you have continuous bleeding and spotted so in my opinion you should consult a Gynecologist and get yourself examined and ultrasound pelvic done to rule out fibroid, polyp and endometrial hyperplasia.
Hope found useful.
Good luck.

I have already done all of that. Pap and cancer screening all ok. Vaginal ultrasound showed no problems. I am not having continous bleeding. Mirina out end of october. Bleeding like a period for the second time in two weeks.
Brief Answer:
Could be due to hormonal changes.
Detailed Answer:
Hello ma'am,
Sorry for the delay in my response.
If everything is fine and you are bleeding like proper menses it could be due to hormonal changes occur near menopause.(considered normal).
So no need to worry but once you will stop menses for one year and then if you get spotting and bleeding that needs evaluation.
Hopefully found useful.
Good luck.
