What Causes Heavy Vaginal Bleeding With Clots Despite Taking Provera?

Question: I have been having perimenopausal symptoms and started bleeding abnormally heavy, passing large clots occasionally for 10 years, getting worse over the last year. I have had cysts on my right ovary, documented by ultrasound since I was 18, now 48. I have 3 children. Two years ago, I started having large cysts in my breast, improved (decrease in size) by my recent use of Provera. My Dr. put me on Provera for 3 months, 1x/day starting with day 19-29 which should have stopped the thick lining from building up, but instead, it gave me my period, first starting with spotting, leading into flooding BEFORE I got my period when I stopped the medication on day 29. Now, they want to start me on Aygestin 3 days-4x/day weaning down from there. I have read the side effects and have grave concerns. While on the Provera last month, month 2, I didn't stop the medication inducing period and I had lost a substantial amount of blood. Naturally, I am apprehensive and scared. If I am flooding now and when I stop the Provera, how bad is it going to get? As bad as last month with an ER visit? Since I don't have many side effects on Provera, (My right kidney is sore after 4 days today of Provera this month, mentally feeling better- less foggy, less anxious) is that any indication I will I be able to tolerate the Aygestin well? Either way, I have to stop the Provera today, so the blood can clot. Will the Aygestin, with all the awful side effects work, or should I just bail on this "therapy" as not for me?

Since you are having a heavy bleeding with clots even while having Provera, you need a stronger hormone preparation to take care of the overgrown endometrium. Continuing Provera is not an option. Taking Aygestin is not likely to cause any more or stronger side effects than you are already having.
It can increase the nausea and headache to some extent but it is worth a try. In case it does not help the n you can consider going for an endometrial ablation after consulting your gynecologist for a symptomatic relief.

I started 4 days out of 10 days of Provera. My dosage of Aygestin is 5MG at 3 days 4x/day, 3 days 3x/day, 5 days 1x/day. Should I drop off four days at the end to make up for what I started with on Provera? When do I start Aygesrin next month? Day 19, counting from where? Is there any concern about the pain in my right kidney? I had a serious kidney infection at 18 that I was hospitalized for. I don't know if this is just water retention or any other concern with this medicine. Any insight? Thank you, you have reduced my fears.

Hello.
Thanks for writing again.
You can start with Aygestin and take it for full 11 days. After you stop taking Aygestin you will have a withdrawal bleeding. The starting of proper bleeding should be considered as day1 of the next cycle and then start Aygestin again from day 19 in the next cycle.
The pain in right kidney is not related to this medicine. It can be due to a small renal stone. An ultrasound scan can help in proper diagnosis.

First of all, thank you, thank you, thank you! After 4 doses, the bleeding has slowed considerably, although I may need a few more doses to cease completely. Instead of depression, I have had NO side effects, in contrast, I feel euphoric, slept well and congestion has cleared up. although this may not last, should I consider a bioidentical progesterone for daily use? Would this work the same? and what are the problems with continuing progesterone after menopause? Thanks again!

Bioidentical hormones are not needed.

If this has helped in stopping the heavy bleeding then there is no need to take any bioidentical hormone for a daily use. You need to use the hormones cautiously as you have a history of breast cysts.
You can continue with Aygestin till you attain menopause. You can continue progesterone after menopause but that needs a few factors to be considered and needs regular check ups and blood tests to rule out any side effects. Do consult your gynecologist to rule out any contraindicating factors.
