What Causes Hematuria While Suffering From Leukocytoclastic Vasculitis?
Question: Hello Doc, I am suffering of Luckoclastic Vasculaties from last 2 month. one month back my RBC count in urine was 15-20, after 10 days it become 5-7. Today i have done again & it shows 40-60 RBC/HPF. I am taking steroid drug( Omnacortil-30MG). My age-49, Blood pressure Normal, No Sugar problem.
Pls suggest.

It seems like an autoimmune disease. Have we done Ana and XXXXXXX antibodies?
In any case, as the RBC in urine has increased recently, I think you need higher doses of steroids. From 10 you have come to 30, was it hiked only recently?
If required, we can add other immunosuppressive drugs like cyclophosphamide, azathioprine or rituximab.

I have attached the ANA & XXXXXXX report. C3 &C4 report also attached
I also done the blood report of CYROGLOBULINS PANEL, Report not received.

this appears to be ANA and XXXXXXX negative vasculitis. Cryoglobulins to be checked definitely. Also serum protein electrophoresis, hep B and C tests and echo may be required to look for potential causes. Also exclude any recent drugs taken which might cause hypersensitivity vasculitis.
Now steroids can be increased further if required then slowly reduce if symptoms controlled.
If required still, antihistaminics and/or colchicine can help. For exact meds and doses, you can discuss with your rheumatologist.

Sir, Today i have received the PROTEIN, TOTAL, 24-HOUR URINE report. I am attaching that report for you.

Protein in urine over 24 hrs is 400 mg per day which is only marginally high. This much is expected considering the RBC s. Red cells carry lot of protein in them, so this is expected. Will not change any treatment but will help in monitoring response.
