What Causes High BP After Having Midodrine?

default
Posted on Mon, 6 Apr 2015
Question: I am a 79 year old woman taking 5 mg of Midodrine 3 x a day for supposedly low blood pressure. But my blood pressure is sometimes over 150 up to 180 and at other times around 120.
In addition I was prescribed 0.1 mg Fludrocortisone twice a day.
I feel very week, dizzy and overall bad most of the day. If I feel fine, I work outside in the a garden mostly on my knees. That makes me feel better.
For me this is very unusual since I was very healthy most of my life.

Brief Answer:
You require dose and frequency adjustments of midodrine.

Detailed Answer:
Hi,
Midodrine is a good drug in the treatment of orthostatic hypotension.

If it is causing shoot up of your blood pressure up to 150 mm hg to 180 mm hg, then you can reduce the frequency of the drug to twice a day as per discussion with your treating physician.

However it might cause rise in systolic blood pressure specially in supine condition rather than in standing condition in which it maintains blood pressure between 120 to 130 and could be the reason of your feeling better while working in the garden.

So, you should monitor your standing, sitting and supine blood pressure timely.

you can continue Fludrocortisone 0.1mg twice a day.

You just require adjustments of the dose and the frequency of this drugs.

The other important thing is you should be diagnosed first for this hypo-tension like dehydration, hypothyroidism or some underlying heart problem etc and for that you will require thoroughly clinical examination by physician.

I also have had a heart examination where I had to carry a shoulder bag around for a day. I also proved negative.

Brief Answer:
To diagnose the underlying cause is important.

Detailed Answer:
Hi,
As i said the important thing is to find out underlying cause of low blood pressure.
The other causes of low blood pressure could be insufficient fluid intake, anaemia, low blood volume, hormonal changes etc.
Apart from medicines, you can also make some life style changes to elevate blood pressure like,
(i) drink plenty of fluids and electrolytes (like sodium and potassium)
(ii) getting up slowly from supine or lying down position.
(iii) compression stocking over legs which reduces pooling blood in legs allowing more blood towards heart helps to maintain normal blood pressure.
(iv) avoiding long standing which might lowers the blood pressure.
Consult your physician for thoroughly clinical examination for the underlying cause and can take above measures to maintain blood pressure apart from medicines.
