Question: I have been dx with hypogammaglobulinemia. I just gave another urine spec for bence XXXXXXX protein and serum for immunoelectrophoresis. One month ago I experienced severe lumbar pain radiating to my hips. Over the next few days I had electric shock type pain going down my legs. Then that stopped and I have been unable to ambulate ever since...my legs are rubber now. Any ideas as to what this is?

Brief Answer:
Systemic disorder

Detailed Answer:
Dear
I am sorry for the symptoms you are suffering
In my opinion bence XXXXXXX protein will be negative
I don't think you are suffering from multiple myeloma because at the 3-4 stage before pathologic fracture multiple myeloma causes severe anemia
Dear It should be a systemic disease so I think imunoelectroforesis will give information
You have done well to schedule the MRI
I would strongly recommend to recheck the periferic nervous system, schedule an ENG
Please test all tumoral markers (such as ca19.9, ca129, alpha fetoprotein etcc)
If you provide me those result I could be more helpful
Anyway I think you are suffering some systemic disorder not a malignant syndrome
Hope my answer will help you in finding the diagnose

I had an EMG which was completely abnormal, as it was 7 years ago. The neurosurgeon said the "disease will be difficult to dx because the underlying demyelinating neuropathy is already present from the connective tissue dz. Also I think I forgot to put Neurontin and Plaquenil down as meds I am taking also. 2 years ago I had a chest CT that showed 1mm lymph nodes in bilat axillary, paracervical, paratracheal, mediasteinal... felt to be of ? significance. Could they play a part in this?

Brief Answer:
Not malignant syndrome

Detailed Answer:
Dear,
It is true that will be difficult to diagnose
Anyway lymph nodes till 1mm aren't pathological
It is something systemic and I think your doctor will go for a lumbar puncture because liquor test can confirm it
Unfortunately 80% of polyneuropathies are idiopathic the cause can't be found ,
It is important to be excluded malignant syndrome
Wish you all the best

Answered by Dr. Eris Ranxha (8 hours later)
Brief Answer:
Lumbar puncture

Detailed Answer:
Liquor test's is a procedure where test's are made on cerebrospinal liquor, meaning the liquor taken from lumbar puncture will be examinated and will permanently exlude or confirm vascular or autoimmune pathologies
Just talk with your neurologist about my opinion
