Brief Answer:
Chlorthalidone and telmisartan are most obvious causes
Detailed Answer:
Dear Sir / Madam
1. The more obvious reason for hyponatremia in the given scenario is use of two drugs i.e. Chlorthalidone and telmisartan (Tab LN trio -20). Concomitant or individual use of these drugs can cause hyponatremia. Chlorthalidone increases renal sodium excretion and Telmisartan can cause SIADH like picture leading to hyponatremia.
2. Beside, Salt losing nephropathy, hyperglycemia, hyperlipedemia, Renal tubular acidosis etc may be potential causes for hyponatremia in such a case scenario. However , no comment can be made about them in absence of diagnostic details at time of admission.
