Brief Answer:
It's due to twincal, adding small dose of beta blocker would be ideal

Detailed Answer:
Hello, I am Dr Mody and I would be addressing your concern.

Firstly it seems odd that the other medication did not work. If at all time permits send me what happened.
Now twincal contains clinidipine (Ccb) , the main action is to relax the peripheral blood vessels, hence decreasing blood pressure which is due to extra constriction of the vessels (simple analogy to try explain you the mechanism), so in such cases there is reflex increase in the heart rate.
So tachycardia (increased heart rate) is a known side effect of twincal.
Now the ideal means here is to add a small dose of beta blocker, best hereby would be bisoprolol 2.5 mg (concor), however keep a tab on blood pressure addition of another bisoprolol can push it down further because of its blood pressure lowering effect, in such case decreasing the dose of twincal to 5 mg for one dose along with beta blocker should be tried.
Let me know if any further clarification needed
