Question: Hi, I have an unresolved hype choice lesion of the left ovary that has been changing size (fluctuating between 3cm & 8 cm) for several months, as documented on pel I ultrasound. Since I was 15 yo I have experienced severe cramping around the time of menstration, so bad I have had episodes of loc. I have been experiencing moderate pain and diarrhea for a number of months, however, only sought evaluation 2 months ago, as I had a mire a iud placed and thought for a bit these things were related to that being placed. My ob dismisses the bowel symptoms as unrelated, although these symptoms almost always occur simultaneously. I have had two pelvic ultrasounds recently and now the radiologist has recommended pelvic MRI, I am 33 yo, have had one pregnancy (carried to term), and have a family history of colon cancer. I also had a chocolate cyst removed in 2008 that was 6cm, same ovary. Im concerned about endometriosis or even this putting me at an increased risk for ovarian ca (some family history of ovarian ca and early menopause- cousins, not mother) I was hoping for some direction on what course of action I should discuss with my doctor, I'm feeling as if he isn't concerned.

I have never been officially diagnosed with endometriosis. Only that doctors have 'wondered' aloud if I could possibly have.

Endometriosis presents as follows:

1. Pain right before and during your period, often severe
2. Usually beginning at a younger age - teens
3. Caused by endometrial tissue that makes its way out of the tubes and implants in the pelvis, causing pain as it cycles monthly
4. When it implants on the surface of the ovary, it can lead to the formation of cysts that contain old blood that looks like 'chocolate'

So, you pretty much fit this description. Is the pain that you have monthly mostly on the side of the cyst that they see on ultrasound?

Yes. is it true they woudnt have missed an endometriosis diagnosis in 2008 when I had the cystectomy? It was performed laproscopically. So I should press my doctor on the endometriosis?

I would schedule a visit with your doctor. I would ask them to print out a copy of the operative and any pictures from the surgery to review with you. You should ask about whether or not the cyst was related to endometriosis.

But regardless of that discussion, the fact is that you have recurrent findings on ultrasound AND pain. You are probably headed for another laparoscopy. At the time of that surgery, consideration should probably be given to removing that ovary and treating endometriosis if present. Sometimes a medication called Lupron can be used as an alternative to, or in addition to surgery to achieve a better long term result.
