What Causes Inflamed Itchy Penis Shaft After An Unprotected Sex With HSV1 Positive?

Question: Around 5 months ago I had brief unprotected oral and vagina intercourse with a woman hsv1 positive orally. She had no cold sores or lesions during our encounters.

Two weeks after encounter i experienced a tingling sensastion on my penile shaft. A small area on the side of shaft became inflamed and itched upon arrival and during. At no point did it blister or form vesicles. It looked exactly like the photos attached. Lesion went away after 4 weeks.

Fast forward 5 months to the present, i have taken HSV1+2 IGG blood tests at 9 weeks, 12 weeks and 16 weeks, ALL negative for HSV1 +2. However, the same itchy lesion that I enountered after my exposure to that woman has arrived again in the same exact spot. This to me seems to indicate herpes. It itches, but is not on a red base, does not have vesicles or an ulcer, BUT it is in the same place. Please help!!

Is this a wart? Why does it come and go? It's been 3 months lesion free and now it came back. I did shave my penis the night prior? Could this cause the same itchy lesion to come back?

I have had other disturbing symptoms prior to the present, but all HSV swabs and blood are negative through 16 weeks. I had these lesion swabbed yesterday, but the PA available said it didnt look like herpes, but had no answer for a diagnosis. Please advise what this could be. Herpes?

Brief Answer:
Possibly due to contact allergy.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for sending your query.
I saw the pictures sent by you.

Without the appearance of vesicles or subsequent ulcers, there is no possibility of Herpes infection. All your laboratory tests were also not supportive of Herpes.
The pictures sent by you also seems to be due to some contact allergy either due to condom or due due to some applications like soaps or antiseptics. That might be responsible for your symptoms.
Otherwise mere phobia or fear towards STD can cause also such symptoms. Nothing to worry. Apply some bland or mild potent steroid creams you will be alright.

Dr. My concern is that I have read that herpes can present itself in many ways and can have atypical presentations. This, along with the fact I have had this lesion exactly in the same spot 3 months prior makes me believe this is a herpetic lesion.

I also had a similar lesion in between my buttocks and popped like a normal pimple 3 months ago, and then resurfaced again in the same spot last week. Its too coincidental for me to have 2 lesions in the exact same spot a week from another.

The lesion in the photo was swabbed via culture but my doctor advised with no liquid or fluid it will be negative. He also advised it did not look like herpes. Does fluid need to be present for accurate result? However, what else can present itself this way 2 TIMES in the SAME location. It also itched, but has since stopped. Hpv??

Can I trust a IGG anitbody test at 16 weeks? If this is swabbed negative, should i trust this result? I got it swabbed with 4 hours of it appearing.

I need assitance understanding what could cause a lesion to appear twice, same spot, if not herpes. Does herpes present itself like this? ?

Brief Answer:
Possibility of nonvenereal skincondition-Psoriasis

Detailed Answer:
Hi,
Welcome back.

Of course Herpes can be presented in many ways and mimicking other diseases
. It is true but the ulcers following the vesicles would behave like this, not in any other ways other than vesicles and subsequent ulcers.
Recurrent spots like this can occur with certain other non-venereal skin conditions like psoriasis, Lichen planus etc.
Ig G for HSV at 16 weeks is not reliable beyond doubt, but along with a negative swab test it is highly reliable. Swabs taken either from vesicular fluid or from the base of the ulcer. If there was no vesicle or ulcer then swab can not be taken from a dry lesion.
So your lesion most likely to be due to a non-venereal skin condition for which it is better to consult a Dermatologist.

Dr. Please review my final and most recent pictures of my lesion. I decided on Sunday night to manipulate and squeeze the soft/medium hard bump very hard. I did this until a small amount of white stuff came out, almost like a pimple being popped. It was very little. Then a lot blood came from the pore I opened up. The pore i broke open is the dark spot that is healing.

Today is Tuesday and I woke up with the lesion almost conpletely gone after applying a prescription antifungal cream. Only thing left is the small pore that will scab since I squeezed it.

The lesion never hurt and has now healed basically within 3 days. Does herpes act like this? I hear recurrent herpes can heal much quicker than a primary outbreak, which could be my case because this same lesion took much longer to heal the first time. I also saw some mild herpes cases online that resembled this lesion. Im still concerned this is a herpetic attack.

How confident are you herpes doesnt act this way in your experience? I was told from a doctor once that not all herpes lesions form blisters and/or ulcers. They can present at times as mild redness, a single bump that comes and goes, or confused for insect bites.

What are your final thoughts here? Lesion that comes and goes in the same spot, itched, and healed within 3 days upon squeezing. Why isnt this herpes in your mind? Because no vesicles? Are vesicles always in the presentation? Thank you!!!Please advise.

Brief Answer:
Unlikely to be due to Genital Herpes

Detailed Answer:
Hi,
I am able to see a healing wound on the shaft of penis.
As you pointed out some white stuff coming out on squeezing, it could be a furuncle or a hair folliculitis. Any lesion after expulsion of the content will heal fast. I don't think it could be a Herpetic lesion. Herpes group virus can produce 3 types of lesion over skin. Herpes zoster, herpes labialis and Genital herpes. Genital herpes always present with vesicles (sometimes missed to identify) and ulcers.
