What Causes Infrequent Episodes Of Hematospermia In A 38 Year Old Man?
Question: Hi doc - I've had infrequent episodes of hematospermia, generally if there are 3 or more ejaculations. 3 years ago I had laprocopic surgery for inguenial hernia. Since I'm getting married I have had an STD test but all is clear in May but all is clear, except from cholesterol at 210. Not really sexually active other than making out with the GF and occasional masturbation (5 times a year). Yesterday had heavy ejaculation after making out. Masturbated once afterwards at home and noticed blood. Last time I masturbated was 3 months ago. Apart from this just nocturnal emissions but the residue doest have blood (generally once or twice a month). I generally exercise walk about 30+ miles per week, drinks lots of coffee and water. 152 lbs 5'11. I had laparoscopic inguinal surgery 3 years ago. Would this be part of the issue? Concerned on whether this affects fertility

- This is not related to Lap Hernia repair.
- This usually does not affect the fertility but surely can be confirmed by semen analysis/tests.
The most probable reason here looks to be congestion of the prostate due to least /very occasional ejaculation. This causes the congestion of the prostate gland and hence the bleeding.
Other possible causes can be bleeding anywhere along the tract; from testis, epididymis and vas deference
This can be diagnosed by the following:
Clinical evaluation and examination by a General Surgeon or preferably by an Urologist.
Per-rectal digital examination.
Ultrasound of the scrotum and the whole abdomen, particularly the Uro-genital system.
Tests of semen particularly for infection.
It is difficult to find the cause of hematospermis in most of the cases but still the exercise is very important to rule out or confirm the causes.
