What Causes Intermittent Lower Back Pain While Suffering From Lumbar Disc Herniation?

Question: Hello Doctor,
My wife has been suffering from disc bulge in L4/L5.
she is taking physiotherapy for the same. (Pain in her lower back is on and off but nothings that disturbs day to day activities)

for last two days she has been suffering severe back pain that she is not able to walk or sit normally.
two days ago she was involved in strenuous activities (long hours of cooking and sat continuously for 4 hours without proper support to her back) compared to the other days.
for last two days she is also having gastric troubles (passing gas)

as we live in germany, we went emergency room. she was given a injection of dexahexal im. and advised to take some painkillers (ibrufin).

its almost 12 hrs that she was given the above injection but can't notice any improvement.
attached the report from emergency room (its in german though)
could you please advice what we should do (we did'nt get any further instructions from emergency room) like which doctor should we visit, any precautions etc...

4)Tab. Gemcal(calcium,vitamin D) 1 Tab twice a day.
