Brief Answer:
May be due to high levels of uric acid and needs diagnostic test for that!!

Detailed Answer:
Thanks for the consultation at my virtual clinic!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

I feel sorry for the painful situation you have and happy to extend my expertise towards your health issue.

PROBABILITY:-
The pain you described is very typically seen in case of high uric acid levels in blood. When it happens, there is accumulation of uric acid in the joint spaces, in the form of crystals.

DIAGNOSTIC TESTS NEEDED:-
CBC with ESR and Differentials.
Fasting lipid profile.
Fasting blood glucose levels.
Serum electrolytes.

Please upload the blood work reports when you get them for more personalized management plan. Meanwhile take Ibuprofen by mouth and apply Bengay Ultrastretch ointment on the painful area and massage gently. Also apply heat pad for soothing effects.

Dip your feet in Lukewarm water with some salt in it for almost 15 minutes. It will give you soothing effects.
Take plenty of clean drinking water.
More vegetables, fruits and salads. Least protein diet. No processed red meat. You may take seafood as proteins like Salmon fish.

Would this be a early sign of a diabetic? No prior family history. Is this caused by hormonal changes or poor diet? Is this typrically common in 25 year old men? I have had no radical changes in my livestyle so it is concerning.

Brief Answer:
Though less chances but we have to get you screened for Diabetes too!!!

Detailed Answer:
Welcome back and thanks for the consultation at my virtual clinic!

Less likely to be diabetes but still it is better to get you screened for Diabetes.

Diet containing protein rich products like meat, dairy, eggs and other proteins may lead to increased uric acid and that may cause pain first in the big toe usually but other toes may also be involved. And yes-it is common disease.

Kindly go for the labs and attach the reports. Meanwhile do the remedies as aforementioned.

DIAGNOSTIC TESTS NEEDED:-
Serum Uric acid levels.
CBC with ESR and Differentials.
Fasting lipid profile.
Fasting blood glucose levels.
Serum electrolytes.

Also give me history of pain as there is any event of physical injury to the foot??
Since when have you been feeling this pain??

As far as pain goes, i felt it firat i. The morning aniut a month ago, i thought i was bit by a spider or had a piece of glass in my foot. Hadnt felt anything then this week it has been persistent. Pain lasts about 30 seconds to a minute then subsides. It comes and goes spiratically and never the same intensity or duration. I asked about diabetes because i read up on gout and found some info on diabetic nerve pain. I will get the tests and forward the information, but does strengthen the case for gout or sound more like diabetic nerve pain. I suspect the former, however, wanted your opinion after providing more detail. Thank you for answering so many questions.

Brief Answer:
To me it is more likely to be Gout !!!

Detailed Answer:
Welcome back and thanks for the consultation at my virtual clinic!

It seems more likely to be related to gout. Diabetic neuropathy and pain in nerves as you mentioned is less likely. But as I suggested, you need to go for the screening in anyway.
