Brief Answer:
It could be anxiety
Detailed Answer:
Hello,
Thanks for using Healthcaremagic.
I read your query and understand your concerns.
40 Year young male with body of 21 years with all tests negative and presence of palpitations, difficulty in sleep and inability to find out anything serious despite hospitalization clearly indicates possibility of anxiety disorders.
Anxiety disorders are the only entity which can explain normal tests despite having the symptoms and in my opinion you need to look for a psychiatric consultation to help the young man.
I hope this helps you.
Feel free to contact me if you have more questions.
Thanks and regards.
