What Causes Irregular Heavy Periods With Implanon Removal?

Question: I've had irregular scheduled periods, often protracted (>10 days) with extremely heavy bleeding for the first three days - I need to change within 1.5 hours and have often soiled my clothing. The remaining days are a mix of normal flow or protracted spotting. Can I do anything to manage the flow and reduce the length of my menstruation? My cycles are fairly random, with the last new cycle commencing within two weeks of the end of the previous 10 day cycle.

Additional background information:
A year ago, I removed implanon from my arm. I had been advised to go on implanon to improve the regularity of my cycle and to reduce the number of days for each menstrual period.

I had, up to that point, been on implanon for 12 months but removed it because my first six months were period free, and I bled subsequently non-stop (mostly spotting and regular flow) for the remaining 6 months. Taking an oestrogen supplement did not assist.

My BMI is about 17.5-18.0 on average. I work out 2-3 Xs a week and eat fairly healthy balanced diet with carbs, meat, vegetables, supplemented with a lot of multi-vitamins and anti-oxidants. To combat the risk of iron deficiency, I eat lean red meat. I have been taking turmeric as a supplement. I am between 40-45 years old.

Hello mam,thanks for trusting health care magic.

It is common to have such pattern in perimenopausal age group but in your case it cannot be just related to age only.
I would like to know certain things -
- if possible please attach your ultrasound report so that i know tHe number and location of fibroids and also endometrial thickness
- any hormone testing done so far especially thyroid hormone testing.

In my practice i advise my patients hysteroscopic guided endometrial biopsy if endometrial thickness on ultrasound exceeds 4 mm.

But if endometrial thickness is less than 4 mm or patient is not willing for hysteroscopy then i put them on oral contraceptive pills after proper examination of pelvic organs and blood pressure.
Oral pills will help by -
- regularising the cycle
- decrease blood flow
- hormone support to genital organs.

Also i would like to suggest you some dietary tips
- increase water intake
- avoid excess caffeine and hot climate
- eat nuts and especially almonds and walnuts daily
- include soy proteins and tofu in your daily diet

Thank you for the reply.

I don't gave the ultra sound reports unfortunately at this point but my hormonal tests were apparently indicating normal ranges when they were done about a year back.

Water - I drink about 1.5 liters of water and lemon a day. There's relatively limited caffeine in my diet unless I am out. I don't drink anything sugary drinks, not even fruit juice but I drink alcohol (wine, champagne) moderately.

Heat - unfortunately I live close to the equator but dress appropriately for the climate.

Soya products - I used to do more but limited my intake. I understand why you are suggesting soya based products.

Walnuts and other nuts - I eat these once in a while. I also take a triple strength salmon based omeg oil with cq10 daily. For cooking and salad oils, I only have olive oils, cold pressed coconut oil. I eat avocados periodically too.


Oral pills - when I was on the estrogen pills for two weeks to balance the progresterone implant, I had extreme fatigue and started gaining wait withing three weeks. I felt really sick and tired for six months there after. Is there anything else you can recommend?

Firstly i would like to appreciate your daily diet schedule and your insight of your medical condition which is a delightful experience.
The weight gain you experienced was because of the continuous progesterone supplement from implant all the time along with cyclic estrogen, but if you start with low dose oral pills it wont happen because in pill free week your body will be free from progesterone.
Before that get a pelvic ultrasonography done because at the age of 43 years this abnormal bleeding pattern can be due to malignant changes in the uterine lining so should be evaluated to the maximum with regular follow up.
