What Causes Irregular Vaginal Spotting And Back Pain While On Mirena?

You can still have some irregular spotting with Mirena
Detailed Answer:
Hello, and I hope I can help you today.
The Mirena usually makes you have very short, light periods or none at all. However even if this is your second Mirena, and you haven't had a period in years, you still can have one occasionally. This may be accompanied by menstrual symptoms like cramping and backache. It does not mean that the effectiveness has been reduced, or that there is a complication with the IUD. Other gynecologic problems, such as growth of a fibroid in your uterus, can also give you back pain and a change in your "cycle"
The first step is to double-check that the Mirena is in place by feeling for the string inside the vagina. If you cannot feel the string, you should see a GYN doctor to check an ultrasound to make sure the Mirena is still inside and in the right position. If it has been a while since you've had a gynecologic examination, it may be worth getting evaluated anyway, just to make sure that there is not something wrong with your uterus causing your symptoms.
I hope I was able to adequately answer your questions today and that my advice was helpful. If you need anything further, do not hesitate to contact me.
Best wishes, Dr. Brown

Then to be on the safe side you may want to have your GYN check it
Detailed Answer:
Hello again,
Well if you can't normally feel your strings, see how you feel for a few more days. There is nothing dangerous that could be going on unless you develop fever, abnormal vaginal discharge or severe abdominal pain. If the back pain stops once you stop bleeding, it probably is just a period. If you continue to spot more than a week and your pain increases or doesn't improve, it might be a good idea to get a physical examination so your doctor can check the position of the IUD. In the meantime, taking an antiinflammatory pain reliever like ibuprofen or naproxen may help.
I hope you feel better, and that this information was helpful.
Sincerely, Dr. Brown
