I have gone through your query and I have also reviewed the photograph.
In the photograph I can see that the skin of the left scrotal sac is dry, thickened and dark.
This condition is known as Lichen Simplex Chronicus (LSC). It is self perpetuating condition. Scratching thicken the skin, this thick skin generates more itch and the desire to scratch which in turn thickens the skin further, which in turn generates more itching. This it becomes a cycle, itch scratch itch cycle.
I usually suggest my patients to use a mid potent steroid e.g mometasone furoate 0.1% twice daily along with an oral antihistamine e.g cetrizine 10mg once daily.
This may be continued for 3-4 weeks. The condition would decrease significantly by then and it can be reviewed then regarding the future course of action.

An OTC oral antihistamine e.g Cetrizine 10 mg will provide you symptomatic relief from itching.
You may use lukewarm water for cleaning. this area.
The cream you have been using is an OTC antifungal, whereas this is not a fungal infection. So this cream won't help much.
You must visit a dermatologist in your region. Your dermatologist will write you one of the prescription steroid e.g mometasone furoate 0.1% cream or betamethasone valerate cream.
These should improve your skin condition significantly, within 2-3 weeks.
