I am sorry to say that the pictures, you have posted is not of very good quality and are taken from very close distance.

1. Total duration of skin rash
2. Anybody in family /roommates/colleagues having similar itching or rash?

past 8 months 7 months anti fungal were prescribed last month scabies treatment is prescribed.
2. Anybody in family /roommates/colleagues having similar itching or rash?
no one have this problem
3. Is itching more during night?
its very high during night and day
4. Any itching over genitals?
yes high on the buttoks and the space between 2 buttocks
Oral Ivermectin, Itraconazole, topical permethrin and antifungals
Detailed Answer:
Considering the history, I would like to treat you for both Scabies and Tinea.
I usually suggest my patients with similar complaints with the following line of management. Please discuss with your dermatologist about these drugs and get them prescribed accordingly.
1.tablet Ivermectin 12 mg once every week empty stomach for 3 weeks.
2. Applying Permethrin 5% cream cream below neck onwards to cover whole body. Washing it off with lukewarm water after 8-10 hours. Repeat the above procedure after 15 days.
3.The key to successful treatment of scabies lies in treating everyone simultaneously in the family and others who are in close physical contact.
4. Also,oral Itraconazole 200mg once daily for 3 weeks.
5. Applying a cream containing betamethasone and sertaconazole twice a day over affected skin for 10-12 days and after that switch over to plain sertaconazole cream till next follow up.
6. Taking antihistaminics as advised would help to relieve itching and irritation.
7.Avoid sharing of clothing and toiletries and launder all you bedding and clothes in warm water.
Do get back to me after discussing the same with your doctor and let me know about the improvement.
