Based on your query, my opinion is as follows:
1. Excessive muscle relaxation of various systems is the cause.
2. Usually a features of opioids, so your doctor is right in thinking of morphine overdose.
3. Biliary duct slowing has resulted in obstructive jaundice features, which is shown through itching. Other system slowing has resulted in straining for urine or defecation.
4. Need to get dose reduced to, at the earliest.
5. Enlarged prostate, would have caused obstruction and straining only for urine. Its not prostate alone, for all these symptoms. If there is any change in food nutrition, fiber and hydration, then other things can be thought of.
For eg: you are not taking in food adequately, which can explain - loss of energy, affected bowel movement (due to low calorie and weakness), and also shaky muscles on mild exertion. Low hydration can explain - loss of urination. Dryness of skin of face - can result in skin itching.
6. I would think of morphine at present. As you have an appointment with the pain clinic, it will be evaluated. If its normal
a. Look into nutrition and high fiber diet - Bulking laxatives maybe taken.
b. Good hydration - At-least 5-6 liters of water/day
