You have been experiencing pain on ejaculation and an itchy sensation in your penis. My first inclination is to be sure you do not have a sexual transmitted disease. It is not uncommon for sex partners to have been together for a long time and then develop symptoms of an STD. the most common infection would be chlamydia. Therefore I recommend that you and your partner both be tested for STDs.
If you don't have an STD then most likely you have prostate infection known as prostatitis. This is very common in sexually active young men. We do not know exactly how you get infection in the prostate but we do know it can start spontaneously. The treatment is usually a month of oral antibiotics. It needs to be a month because the penetration of antibiotics into the prostate is low.

You may still have an STD. I would still put chlamydia high on the list for possible causes for the burning and itching. I would recommend seeing your doctor and have the tests run to be absolutely sure. Your girlfriend may have chlamydia and not really have any symptoms.
This does not sound like prostatitis as it would be a deeper pain and burning
