Detailed Answer:
Hi,
Welcome to HCM...
You said you have itchy reddish spots with fissures on foreskin of penis. And that is increased after taking wine. I can understand your concern for relapsing skin disease on glans penis and prepuce. To come to most likely diagnosis I would like to have answers from you and to see the photo of affected area.
- from how many days you have this problem?
- are you married?
- have you physical relationship with anybody?
- do you have burning, itching or pain in affected area?
- please describe the lesion?
- any other internal disease like DM,HIV,blood diseases or other?
- past history of any disease?
- do you take any medicine?
- for how many days you apply fucicort cream?
Please answer above queries and do not forget to post the photo of lesion. After seeing the lesion, I would surely give you perfect advice.
Awaiting your reply...
Thanks.

Candidal balanoposthitis
Detailed Answer:
Hi,
Welcome back...
Thanks for answers. But, you did not send photo of the lesions. If you are not able to upload, please mail them as attachments to YYYY@YYYY with Subject Attention: Dr Ilyas Patel.
After reading your answers, I deeply feel that most probably you may be having fungal infection of glans and prepuce. It is called candidal balanoposthitis. It is caused by fungus called Candida albicans. It may be transmitted by sexual contact. It is commonly found in persons in patients having poor hygiene and poor immunity. You consult dermatologist to have firm diagnosis and perfect treatment. But remember without seeing anything nothing can be confirmed.
I would like to suggest as follow...
- antifungal cream like clotrimazole cream application twice a day
- antifungal like ketoconazole tablet or itraconazole cap for about two weeks
- Sitz bath.. in dettol containg water tub daily
- to improve immunity..have nutritious food, do exercise...etc
- to keep hygiene good
- have protected sex
I hope above hints may be helpful to you.
If you have any more query, you may ask.
Thanks with best wishes..

Pearly penile papules & CBP
Detailed Answer:
Hi,
Welcome back..
Lot of thanks for emailing photos of affected parts. There is a clear diagnosis of pearly penile papules along with candidal balanoposthitis.
Pearly penile papules are small dome-shaped to thread-like skin-coloured bumps that are typically located on the sulcus or corona of the glans penis. The papules are commonly arranged all the way around the head of the penis in one or several rows.
The lesions may be anatomical variants. The lesions are benign and harmless. They should be distinguished from molluscum contagiousum, warts and Fordyce spots.
Pearly penile papules remain harmless lifetime,so no treatment is required. But if you feel embarrassing, the lesions may be treated by electrocautery or cryotherapy. For that, you consult dermatologist and have treatment.
For Candidal balanoposthitis, you are taking right treatment, so continue till you feel better.
I hope this would help you in managing your skin problem. Still have a query, you are welcome.
Thanks with best wishes...
