What Causes Itching Under The Penis Foreskin?

Question: Hello i have an itch under my foreskin which has been there for over 4 months. it can be uncomfortable when touching but for majority of the time it goes by unnoticed. during sex it can be a little sore. i have tight foreskin which doesnt go back when i am erect so i only see this when i am showing etc.

Welcome to the forum.
I had a good look at the photo.
It looks like infection of the area - most probably bacterial or fungal.
The following things should help.
1. Cleaning the area during shower with soap and water.
2. Application of antibacterial/ antifungal ointments thrice a day for 3 weeks.
If repeated episodes occur, it can be cured by circumcision.

thanks for quick response, so i have had cleaned the area every morning and night and seen no improvement.

my concern is a STD. if it was a STD would it have spread to other areas

Some STD can spread.

It does not look like STD to me, as they appear different.
If worried, the battery of tests needed to exclude STD may be done.
Some STD like syphilis an spread to other areas.
