Greetings from health care magic!!! I am Dr Thakker and shall be answering your question.
I would put forth the following differential diagnosis for itchy groin:
1) Fungal infection secondary to tenia cruris; commonly known as jock itch
2) Candidial intertrigo, more common in diabetics though. Is your friend suffering from diabetes?
3) Contact dermatitis
4) Phtiris pubis/ louse infestation.
I would suggest an antifungal ointment to be applied over the itchy areas along with hydroxyzine at night time to prevent itching. Local examination of the area will be very helpful to clinch the right diagnosis.
