Brief Answer:
Dryness / allergy / infections...

Detailed Answer:
Hello,

Itchy nipples could be because of a lot of conditions. Most commonly it is because of dryness. It can also be due to allergy to clothes especially synthetic fabrics. If he has itching elsewhere like underarms, groin, around belly button, wrists and hand it could even be due to scabies. Sometimes certain creams or lotions can also cause allergy.

I will advise him to apply petroleum jelly 3-4 times daily over nipples. You may also apply hydrocortisone cream twice daily over nipples. I will recommend a cotton vest underneath before wearing shirts. If itching is not coming under control even after 1 week, you should consider consulting a dermatologist.

Hope this helped,

Take care
