Posted on Tue, 10 Feb 2015
Question: i went to a walk in clinic yesterday for a yeast infection.. I saw a nurse practitioner & she prescribed flucosinole & Nystatin cream. I have an itchy rash on my privates and inner thighs. It is also swollen. The swelling has traveling further down my thighs. Am I expecting too much? I thought I should have felt a little relief by now. Could it be something else.

Brief Answer:
does sound like a fungal infection

Detailed Answer:
HI, thanks for using healthcare magic

The swelling is part of the inflammatory response to the infection.

From the description it does sound predominantly like a fungal infection which should respond to the nystatin. The steroid would help to reduce the inflammatory response.

It can sometimes take 24 to 48 hours for any initial improvement especially with a fungal infections. Fungal infections are slow to respond to treatment and can sometimes take 2 to 3 weeks for complete resolution.

Because of the itching, it is possible to introduce bacteria into the fungal infection , a person can have a fungal infection with a secondary bacterial infection.

If you do not notice any improvement at all in the next 24 hours then this is a possibility which means that you would also need antibacterial treatment as well.
During this time, along with the treatment given, you may want to consider doing the following:
(1)add an anti inflammatory pain killer. Even though there is no pain, the anti inflammatory aspect may help to reduce the swelling

Eg- ibuprofen, cataflam or naproxen

(2)the folds of the skin need to separated. Moisture, heat and friction are the cause of the rashes in this area and other similar areas (axilla, under breasts)

Using cotton or linen clothes to separate the folds or using barrier protection creams such as zinc based creams may help

Wear cool clothing, no restrictive clothing

I hope this helps, feel free to ask any other questions

thank you. The ibuprofen seems to have helped the swelling and redness is starting to disappear, but my genital area and inner thighs still itch. the Nystatin cream does not seem to help the itch. What is a zinc based cream? Will that help with the itch? The itch seems to be worse after I urinate. Can HealthCare Magic prescribe drugs?

Brief Answer:
steroid cream would help the itching

Detailed Answer:
HI

A zinc based cream is for barrier protection. It would prevent the folds from rubbing against each other (friction), reduce moisture, also prevent contact from anything else.
Friction and moisture will worsen the rash or delay healing.

The steroid cream that you mentioned you were given along with the nystatin should help with the itching, if it is not then you may a stronger cream.
Nystatin is antifungal and would help in the general improvement of the rash.

No, unfortunately cannot prescribe for you.
