Question: My boyfriend has had a rash for month or more now. He has been put oon steroids which seems to help but it doesn't cure it. We have eliminated anything and everything we thought could be causing it. the rash is circles with pin holes in a circles with one pin hole in the center. it's very distinctive and the doctor is unaware of what this is. At one point his lips and eyes were swollen and the rashh seems to be reactive to pressure. for instance, when a ladder pressed against his waist for a period of time the rashes broke out along that line, same as when carrying heavy items on his shoulder. They are itchy.

Pressure urticaria is a type of skin reaction / allergy where the skin swells up after application of pressure, either immediately or after few hours. ( when it is called "Delayed pressure urticaria"). This swelling lasts for a few minutes to hours, typically subsides within 24 hours and appears elsewhere on the body on areas where pressure has been applied.
There is no specific cause for this. Occasionally, it could occur as a reaction to a particular food, drug or infection. It subsides spontaneously on its own within a few months or sometimes years.

Are the rashes he is having transient? How long do they last without any medications?

Has your friend introduced any new food item ( including health supplements like green tea) or is he on other medications?

The other possibility I would consider is another type of skin reaction called as "Erythema annulare centrifugum". This is being considered only based on the description of circular lesions given by you.
