What Causes Itchy Scrotum And Bumps On Shaft And Foreskin?

Question: Hi,

A few weeks ago, I noticed that my scrotum was really itchy. Then a week later, my foreskin and part of the glans were itchy as well(usually in the shower or when in bed). Then a days ago, I noticed some pimple like bumps on my skin of the shaft and foreskin. There used to be just 2 or so and now there's about 10, and there seem to be something blackheads on top of the bumps. It used to feel mildly itchy but now I feel some pain. There also seems to be lumps on the left side of my scrotum. I was scratching it one night and then I saw there were lumps that are like swollen scrotum skin. It used to itchy but now its painful when I put my legs together when it touches the spot. The pain feels as if someone is putting a needle at the spot. Another thing I noticed is that my lymph nodes(or I think it is) hurts when I press down on it. This happened before I got the bumps on penis and scrotum. I've never had sex before. What could this be? A few days later, it hurts to walk because of the friction caused by the legs rubbing the scrotum. It helps if I put toilet paper on the bump on the scrotum Thanks!

I can understand your concern for small pimple like bumps on penis and scrotum.

There can be multiple possibilities of the bumps and as per your history it appears to be "Scabies". It is a mite infestation which gives typically itching at genitalia, web spaces and other folds of the body. The itching is typically more pronounced in bed at night time.

To diagnose the exact entity I request you to kindly upload few good resolution pictures of the affected area so that I can analyze it better.

Kindly send good resolution pictures focusing the area of concern which can be seen on higher magnification. For uploading the pictures you have a facility on the right side of the page or can send on:

Hi Doctor,

Thanks for replying. Pictures have been uploaded. Please ignore the white stuff on the scrotum(toilet paper) I noticed last night that my foreskin is swollen or its the bumps that are making it big.

Here are the answers to your questions:

-the area doesnt seem to itch as much but now it hurts when something touches it(includes friction from walking between legs and scrotum). I woke up several times last night due to the pain.

-No, the pain/itching is contained to the genitals

-No, there is no family history of itching

-No, I do not share my bed or room with anyone else

-I have phimosis, never had sex

Hello,

Welcome again to the forum and thanks a lot for sending the pictures and other information.

I can find out that all the six pictures are same. I am not able to find out any pictures of the penile area. Anyhow, I can make out that the scrotal skin has few pimple like growth over certain areas. Also I can see that the skin is bit macerated.

In my view I can make out the best possibility of scabies, which I already described.

The macerated changes in the skin are due to alcohol application. I first and foremost suggest you to not to apply any alcohol like stuff or anything which can burn the skin.

For the betterment kindly follow the suggestion below:

- Do cold compression with towel soaked in cold water.

- Keep yourself away from hot and humid climate.

- Take two times bath.

- Wear cotton clothes and cotton undergarments to be changed two times a day. Do not wear any synthetic or tight garments.

-Do not sit continuously for long hours.

- In my patients of your kind of condition I suggest them to apply a cream containing permethrin 5% lotion below neck for overnight period.

- A combination cream of antibacterial and mild steroid can be applied over the macerated area.

- In my patients of your kind of condition I suggest them to take oral antihistamine like loratadine or levocetrizine two times a day under medical advise if there is moderate to severe itching is present.

-Don't try to scratch or rub the area as can lead to further aggravation of the problem.

Hello young buddy,

Welcome again to the forum and thanks for your follow up.

Scabies can be easily treated once you follow the instructions and maintain good hygiene along with a policy of NO SELF TREATMENT (like application of alcohol or anything irritating to the skin). Skin over genital area is very soft and thin, so if you apply any irritant chemical then can cause severe irritation. The pain and prick sensation is because of macerated skin over the scrotum due to application of alcohol.

The permethrin 5% cream is probably an over the counter medicine so you can visit your pharmacy and take proper guidance. The loratadine tablet is available as Claritin and can be obtained accordingly. Both these things can be used under medical supervision to get an early relief.
