Brief Answer:
Most probably colonic infection

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
Read and understood your history of dull pain in left upper abdomen below rib cage that is getting sharper on breathing in. Also have loose stool and gas; going on since yesterday morning.

This is most probably clinical presentation of severe colitis with infection. Gram negative infection does not give rise to fever.

I would advise in such a case a course of an antibiotic, metronidazole, probiotic, antispasmodic for 5 days. This should help you to get a complete relief.

Investigations of stool, blood urine may help as also the ultrasonography of the abdomen and X-ray of the chest to rule out other possible causes or co-existing problems.

I hope this answer helps you, please feel free to ask for further relevant queries if you need to or if you feel that there is a gap of communication.
