Brief Answer:
need CT KUB

Detailed Answer:
Hi,
You certainly do need treatment. An 18mm calculus is very large, and will not come out spontaneously and might even become larger and can cause damage to the kidney. The way to treat this stone will be most likely by PCNL, which is an endoscopic procedure through a small hole in the side of abdomen. This is done after planning by means of either a CT scan of kidneys and bladder KUB which is preferable, which will also inform if there is any other stone in the passage because of which kidney swelling has occurred which is the meaning of the mild hydronephrosis. Because it is mild, it means usually there is not much damage yet to the kidney.
Hope I have been of help.
Regards.
