Brief Answer:
Young child with several symptoms

Detailed Answer:
Hello,

How long has your grandson been ill? Did his symptoms come on all at once or one at a time? Fever? Appetite? Weight loss?
To what extent is the lethargy effecting him? For example, stays in bed all day, is so tired that he does not go outdoors to play with friends?

Does eating relieve his stomach pain or make it worse?
Does he bleed from any other site in body other than nose? Does he have allergies? Bruise easily?
Is he taking any medication?
I am having difficulty associating any illness with all of these symptoms.

Here are some that are found with one or two symptoms:
- Depression with lethargy and stomach pains
- Nose bleeds with allergy, high blood pressure
- Nose bleeds, lethargy, stomach pains with malignancy
- Cold sensation with hypothyroidism

You may wish to take his temperature(underarm for three minutes)2-3 times a day each time asking how he feels. Keep a log.
There are many possibilities as I've suggested many of which are quite serious. Please take your grandson to his pediatrician AS SOON AS POSSIBLE to have a full evaluation.

Hope I have answered your query. If you have any further questions, I will be happy to help.
