What Causes Light Spotting And Lower Abdominal Pulling Pain When With Mirena?

Question: I have had the MIRENA IUD for two years, I had it inserted at my 6week post partum visit after having my son.
Prior to having my son I suffered from endometriosis and have had surgeries for multiple chocolate cysts which had ruptured causing internal bleeding.

I've had no problems at all since I had my son. I've also not had a period, which was normal for me prior to pregnancy as well. I had bleeding for two weeks after insertion and nothing since. About 5 weeks ago I had very light spotting for three days, followed by a pulling type pain on my left lower abdomen... A week later I developed tender breasts, and experiencing the "let down" feeling... And leaking from nipples at night.
I've felt fluttery feeling in my belly, extreme bloating, and have felt and saw the movement on the outside on my belly.
Negative home urine pregnancy test a week ago, negative blood pregnancy test on three days ago. And high end of normal range prolactin level.

All symptoms persist, as well as today I've had a very watery pink discharge

I def. feel pregnant. Could I be pregnant? Is it possible to have a false negative blood test?

I'm scheduled for an ultrasound on March 25th.

Thanks for the query,
It is unlikely that you would be pregnant with MIRENA in situ since 2 years now and with ovulation prevented completely as evidenced by amenorrhoea, there are no chances of pregnancy .
the bleeding that you are experiencing is likely breakthrough bleeding which can occur due to stress related decrease in progesterone levels which are daily released by MIRENA , hence cause a light withdrawal bleed which is pinkish in colour. This is normal and is not an implantation bleed at all. Once the pregnancy tests are negative , it further confirms absence of pregnancy.
Discharge from nipples is due to high prolactin levels and causes for hyperprolactinemia should be searched for like microprolactinomas in brain . This needs A mri scan. If ruled out then you can be placed on dopamine agonists to control prolactin levels and prevent galactorrhea.
you may discuss the options with your physician in detail and take an informed decision,

Is it possible that the hcg level was too low to get a positive bc its too early?
How does hyperprolactinemia contribute to the abnormal bloating? And what are the movments?? I am usually a very thin 110 lbs 5'6" female. In the past month this hard belly has appeared and continues to get larger. My clothes no longer fit properly. I have attached an image of my torso, taken this evening.

Well, hyperprolactinemia doesn't cause bloating.
bloating is caused by the progesterone imbalance. looking at the picture and also with the decree that your gynae felt something per vaginal examination and also with a history of endometriosis earlier, there are chances of an ovarian hemorrhagic /chocolate cyst which is growing in size.
This shall be cleared after the ultrasound pelvis.
Hence please get an ultrasound done and please share the report.

Ok, one last thing. I just checked to feel for my mirena strings, and i am a bit alarmed.
I was able to feel the very tip of the string, but it was off to the side, in a hard lump, about the size of a ping pong ball.
Is this my cervix? And should it be off to the side and hard?
Every time i have checked before i was more in the middle and not firm.

Please advise, do i need to go to the ER?

I am pretty concerned with this.

Yes that hard lump that you felt is the cervix and the thread can be felt at the side of the cervical opening and thats normal indicating that the MIRENA is still in situ.
There is need to panic.
