Brief Answer:
Can be due to hormonal changes or pain killers.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The light periods like bleeding after oophorectomy can be due to decrease in the levels of feminine hormones that act on the endometrium. This bleeding is likely to stop in next few days. The bleeding can also happen in response to the pain killers that are given after the surgery. In case the bleeding is persistent then do consult your gynecologist for a direct examination.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
Regards.

Thank you, Dr XXXXXXX This is not for me but for someone to whom I am providing social services. This man is a transgender man who had a total oophorectomy over five years ago and therefore has been totally over the post-surgical period for a long time. He is afraid to seek gynaecological care for privacy reasons. I wanted to ask on his behalf what could cause the bleeding, as he does not produce estrogen, takes testosterone to have hormone levels that are within standard medical ranges for males, and has no ovaries or menstrual cycle. Could you please answer with a differential diagnosis of what could cause the bleeding in light of these details? Thank you.

Brief Answer:
Can be infection or injury.

Detailed Answer:
Hello.
Thanks for writing again.
Thanks for providing the details. The bleeding is not hormone related. It can be due to infection or local injury. In both the cases a direct examination is necessary to find out the cause. His bleeding profile also needs to be evaluated.
Hope my answer is helpful.
