Question : I feel lightheaded and fatigued after short walks or raising my arms to drive a car. It usally does not occur during my daily 2 mile walk. The light headedness is almost always accompanied with the smell of acetate.

Brief Answer:
Could be cardiac issue but little unusual

Detailed Answer:
Your symptoms could be cardiac; but little unusual that you do not get them during your regular walks.

Since you are above 60 and have previous history of hypertension and you are obese, I would order for cardiac evaluation to make sure every thing is alright. Please visit a cardiologist who can order an EKG, echo and if needed TMT.
