What Causes Long Standing Cough?
TB vs COPD
Detailed Answer:
Hi
I am Dr Mittal
I have read your query
I will try my best to help you.
The problem you have stated could be possible due to a number of causes.
However I would like to focus on 2 reasons which may be the most possible causes of your symptoms. I will need you to read the information provided and provide additional information accordingly in view of the limited data I have available from the query.
First, in XXXXXXX there is always a possibility of tuberculosis. In XXXXXXX a long standing cough with or without fever, with cough often with blood in it could possibly suggest an ongoing tuberculosis. The patient may or may not feel breathless- depending on the severity of infection.
"Pan traas gheyu nako" tuberculosis is fully treatable in almost 99% cases now. the therapy is effective. We will have to get an x ray chest. Ideally I would have preferred a CT chest, but that may be too expensive. With that we will do a sputum test- sputum for AFB staining- at least 3 morning samples. The sputum tests are usually free of cost in nearest RNTCP centre, usually located in the government hospital.
Along with that we will have to consider the possibility of COPD/asthma. These conditions are frequently found in people living in polluted areas or those who are smokers. The patient has sudden cough onsets with white-yellow sputum. There may be usually associated feeling of breathlessness and a whistling sound while breathing may be heard in very severe cases. A chest CT with spirometry are the needed tests for this condition.
"Dogh condition treatable aahe. Manhun traas ghyache nahi. Fakt test XXXXXXX saangayche. "
I have tried my best to answer the query in as simple a language as possible. In case you dont speak marathi, kindly pardon my attempts to make it more conveniet for you.
Best of luck

extra esophageal manifestation of GERD
Detailed Answer:
Hi, thanks for the response.
I think there is a strong possibility that the symptoms Are due to GERD. This happens when you have gastritis. The symptoms of gastritis like abdominal burning or discomfort, nausea specially in the early morning may often be associated.
What happens is that the acid in the stomach comes out of the stomach. The acid then flows up to the throat and burns the mucosa there causing symptoms like cough.
In this case a simple course of proton pump inhibitors may help.
You can try XXXXXXX omeprazole 20 mg twice a day. If effective, I think it would cause relief within 7 days, partial if not fully.
I have tried to make it as simple as possible. I hope that the information helps you. Please feel free to contact us for more information. Best of luck.
