What Causes Loose Motion While On Medication For Paralysis?
phenytoin 50 mg BD
Levitracetam 250mg TDS
Clopidogrel+ asprin 75 mg OD
Atorvastatin 10mg OD
Rantak 150 BD from last 2 yrs she HT Diabetic epileptic
recently she is she is having frequent loose motion from 10 days loose motion stops by metrogyl 400 and again starts in 3 days kindly suggest treatment
She needs clinical evaluation.

Thanks for your question.
Noted your concern.
In such a patient there may be many reasons for loose motions like simple change in food habits, food poisoning, infection, diabetic secretary diarrhea.
So clinical examination & detailed history taking is important.
Also in her case there is a rise in serum creatinine level (which denotes Kidney function) it may be due to dehydration (due to loose motions) or a part of long standing diabetes & hypertension.
So it will be better for her to be seen by physician.
Considering infective diarrhea (as White blood cell count is marginally raised) antibiotics like Cefuroxime and Metronidazole may be useful.
Maintain her oral hydration.

Thank you for ur prompt response.Her RBS is is 169mg/dl the symptoms of diarrhoea relieves after taking metrogy but again appears in 2 -3 days how diabetic Secretary diarrhoea is manged . I have shown to physician he asked to continue metrogyl + oflox for 5days she is currently not taking antidiabetic drug as rise in BS is only from last 15 days previously she used to take it but after septicemia treatment in ICU 2 yrs back her BP & BS was normal for last 2 yrs kindly sugest treatment for diarrhoea and pus cells in urine and white discharge
I m post graduate in pharmacy can correlate the things

Detailed Answer:
Continue Metrogyl & Oflox as advised it will take care of both diarrhea and pus cells in urine.
If despite of treatment pus cells in urine persist there may be need for urine culture and sensitivity testing and change in urinary catheter should be considered.
Avoid intake of high sugar or high fiber load which prevent osmotic diarrhea.
Care should be taken during use of oflox (Quinolones) as they reduces seizure threshold and better to be avoided in old age people and those with seizure disorder.
