Brief Answer:
Multisystem envolvement.

Detailed Answer:
My dear,
Thanks for sending your query to us.
There looks like a multisystem envolvement.Cardiology, neurological and vascular specialists have to have a combined effort to come to a helpful solution.
The data you have provided is not enough.More details of tests are needed. We need to know the reason of surgeries after cataract and reports of neurological examination as well as Fundus examination report to know the reason of visual loss.
Swelling in feet and lack of energy may be due to heart problem, for which he is under good care..
Please get back to me with relevant reports so we can offer some helpful suggestions.
