Question: Hi Doctor,

I am 31 Male, going through low sex drive even after high total testosterone level.

Please provide some guidelines or medicines to restore libido to normal level.

Low libido is caused by a number of physiological, emotional, and lifestyle factors.
Some of them are:

-Stress, it may be social ,economical or occupational

-Psychological problems like depression, anxiety

- Excessive substance use like alcohol,smoking

- Comorbid physical illness like diabetes , hypertension

- Lack of sleep, obesity, irregular diet schedule, more junk food

-Erectile dysfunction problem- due to failure of performance in sex secondary low libido

-Hormonal problem- which you have already sorted out

-Relationship problem

-Medicines going on for other problems

-Busy stressful life


Treatment depends on finding out the cause.

General measures to improve libido are:

-regular sleep, exercises, diet schedule,

-healthy food including fruits and vegetables, low fatty food

-sparing quality time for self and with partner

I would like to brief you on my health problems. Problem occurred due to excessive use of steroids while working out at Gym. Excessive workout and steroids suppressed my testosterone production. Consequently, I have to face this problem:

I am not suffering from listed points.
•     No Stress/Economical or Occupational problem
•     No dibaties/No hypertension (History of high BP when I was on Steroid)
•     No relationship Problem, No alcohol/Smoking, No obesity
•     Taking Good sleep, exercises, healthy food, quality time with partner
•     But, Depressed due to sexual performance anxiety

Below are my responses on each point:
•     I am suffering from lower sex drive/ED from around 8 months. (Since, I am on anabolic steroids)
•     No, Doctor I don’t feel listed cases are applicable in my case
•     Married few months ago, since then surviving on sildenafil/tadalafil tablets
•     Frequency of sex before marriage is once/twice in month.
•     Testosterone injection was given to me to enhance performance in bodybuilding. I took them 2 year ago and possibly my natural testosterone production gets slow/may be shut down. Thus, I have to take 250mg/week sustenon Injection (combination of 4 testosterone).

Thanks for information.Sorry for delayed response.

Cause of problem is injections of testosterone for body building purpose.

This artificial testosterone can disturb hypothalamus-pituitary-axis of hormonal system and causes dysfunction of hormones in the body.I could not find any attached reports. Please upload them.There is need of current serum testosterone, prolactin, FSH and LH level.

Considering your history there is no physical problem related to penis as you are able to perform with tadalafil/sildenafil.Your problem is mostly related to physiological hormonal disturbance which is responsible for your low libido.There is need of solving issue of your natural testosterone secretion.Endocrinologist can help best in this case.

Frequency of sex once/twice in a month before marriage was with your wife or with someone else?Please provide some details if feasible.As your problem has been started after marriage only.

You mentioned that you have performance anxiety.One possibility is you do not have any major hormonal problem also, it may be just your concern and worries regarding sex and making you depressed and low desire of sex.In this sex counseling,couple counseling and learning master and XXXXXXX technique can help.Even low dose anti anxiety medicine like clonazepam before sex is also useful.You need to consult psychiatrist/sexologist for the same.

All reports are almost normal. HDL cholesterol is low which you can improve by taking more PUFA containing food(includes dry fruits,avocado,olive oil,oatmeal and seafood).

There is nothing significant in your report which is associated with your problem of low libido.

Try to treat performance anxiety, spend more quality time with your wife apart from sex and if feasible take her to doctor for couple sex counseling.

As per your analysis it seems like is totally a physiological hormonal disturbance, as suggested; I will surely consult with endocrinologist

Frequency of sex before marriage wasn’t with my wife. Problem was started 6 months ago when I was on steroids (Testosterone injection & Menabol tablets).

Currently, I am working on Kegal Exercise and soon will begin with master and XXXXXXX technique. Also, will let you know about results of suggested anti-anxiety medicine.

Kindly respond my below listed queries:
•     I am taking 50mg sildenafil once in a week, does it have any side effect till I restore my natural testosterone secretion
•     Should I go for Semen count testing to check the fertility or remain focus on ED/lower sex drive problem?
•     Please suggest good endocrinologist in Delhi-NCR region.

Yes you can take sildenafil,semen analysis can be done for confidence

Hi XXXX,

It is good that you are somewhat relax. It will hep .

- If you do not have any significant side effect of sildenafil until now you can continue for short duration(up to 5-6motnhs)without significant problem. Just once go for ECG and blood pressure check up.
But try to take half dose after sometime and check that it is effective or not. By that way you can boost up your confidence.
In long run after very regular intake for more than 1-2 years it can cause cardiac and visual problem in some patients(most of them are in their forties or fifties and with high blood pressure and diabetes, not in young)

- I think your semen analysis will come normal.Just go for that once after 3-4 days of abstinence from sex/masturbation.It will not take much time. You will have more confidence and be relax after normal report.

- I do not know any endocrinologist personally there. But I am sending a link from HCM showing endocrinologist in XXXXXXX You can consult any one of them after searching details.
