What Causes Low Platelet Count Along With Low Monocytes And Basophils?

Question: I am 41 year old male. My blood test indicates platelet count of 150, just at the lower end of the normal range. But, My MPV and PDW are higher than normal. My monocytes and basophils are lower than normal.

What does this indicate ? Is there any cause for worry ? What should be my next step after this result ? I think I have primary hypothyroidism also, since my TSH is 11.88, while my total T3 and total T4 are in the normal range but at the lower end of the normal range.

Good afternoon.
I can understand your problem.

I went through your history and the uploaded results of blood tests including the thyroid profile. It seems to me you have some sort of early nutritional anemia & subclinical hypothyroidism.

1. Red cell distribution width (RDW) is a measurement of the amount that red blood cells vary in size. High RDW means that the red blood cells vary a lot in size

2. Mean platelet volume (MPV) reflects the average size of platelets present in a sample of blood while platelet distribution width (PDW) reflects how uniform the platelets are in size.

3. Platelet counts of 150 (which is at the lower end of the normal range) with high RDW, MPV & PDW are suggestive early nutritional deficiency of Iron, Folic acid or Vitamin B12.

Regarding Thyroid profile results:

TSH is high & T3 & T4 levels are within in the normal range. This is suggestive of a condition called "subclinical hypothyroidism" and it is an early stage of hypothyroidism.

Although this condition may resolve or remain unchanged, within a few years in some patients, overt hypothyroidism develops with low free T4 levels as well as a raised TSH level.

You may need treatment with Thyroxine if you develop symptoms of hypothyroidism like constipation, weakness, cold intolerance, hair fall or premature greying of the hairs.

Dear Doctor,
Thank you for taking the time to review my report and diagnose my case. I really appreciate.
However, you did not advise what should be my next step w.r.t nutritional anemia and hypothyroidism.
1. Page 3 of my report states that Iron/total Iron binding capacity/Transferrin saturation all are normal. Does this mean that deficiency may be that of folic acid or B12 ? How do I find out ? What should be be my next corrective step ? Diet changes or do I need to see some specialist ? if yes, which specialist ? Please advise.

2. subclinicial hypothyroidism - I do not have many symptoms, but yes - a few like greying of hair, cold intolerance, occasional constipation etc. I have been told that once you start thyroxine, you will need to continue it all your life. Is that true ? With thyroxine, is it permanently curable ? What does the available data say about permanent recovery of such cases as mine and in what time period? What are the side effects, if any, of long term use of thyroxine ? Doctor, you may not answer this, but just wanted to know your opinion that is there a homeopathic treatment for my case ?

Thanks for your reply.

The blood picture is showing an early stage of anemia, so nothing to worry about it. What you have to do now is just take a healthy balanced diet including fish, poultry, eggs, vegetables and fruits.

Regarding hypothyroidism the deficient hormone thyroxine has to be replaced for the relief of various symptoms like weight gain, constipation, cold intolerance and greying of the hairs. As long as you take Thyroxine your symptoms are cured.
The common side effects of thyroxine include headache, nausea, increase heart beat, palpitation & sweating.
For homeopathic treatment you may consult the concerned doctor specialized in homeopathy.
