From the description given following possibility needs to be considered
1. Varicocele: It is dilatation and enlargement of veins draining testis. It usually is causing dull aching pain in testis which is aggravated by standing and wearing very tight or very loose underwear.
It can be confirmed by good clinical examination from your Physician
2. Underlying constipation: Reduced intake of water and fiber in diet could lead to constipation and cause lower abdominal pain.
I would suggest you to consult your Primary care Physician for thorough clinical examination and assessment. In the meantime I would advise you the following
1. Proper scrotal support: Wearing the right size underwear will provide support to testis and help relieve pain. Preferable use only cotton based underwear
2. Ensure good intake of water: 2-3 liter a day
3. Get Urine Routine and microscopic examination done to rule out Urinary tract infection
4. High fiber diet: 2 servings of vegetable salad and 2 servings of fruits especially citrus fruits will ensure adequate intake of fiber
