The description can fit a number of conditions but a Urinary tract stone or a Urinary tract infection are likely to be the cause. A musculoskeletal problem would be more evident on the physical exam and would require imaging studies. You can be more sure of the cause once these are worked up. An Ultrasound (Abdomen and pelvis ) along with a Urine R/E should be done. Anti spasmodic ( like hyoscine butylbromide) along with NSAIDs ( like Naproxen or diclofenac) should be prescribed for the pain. PPIs (esomeprazole) and anti emetics (metoclopramide or domeperidone) should be given for the associated nausea and vomiting.
