Brief Answer:
You need to go to ER.

Detailed Answer:
Hello
Thanks for query .

Severe pain in loin and fevers with rigors strongly suggests presence of stone in kidney and needs to get confirmed by following tests however since you have appointment with Urologist after 5 days I would suggest you to visit ER and get following basic tests done
1) Urine Routine and Urine culture
2) Ultrasound Scanning of abdomen or C.T scan

In the mean while start taking broad spectrum antibiotics like Cefotaxime urinary antiseptic like Nitrofurantoin and pain killer like Brufen twice daily along with urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Consult Urologist with all relevant reports .He will decide about modality of further treatment depending upon findings and final diagnosis

Get the prescription of medicines from your family Physician.

Ensure to drink more water .To keep your urine dilute This will help to control dysuria.

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
