What Causes Lower Back Pain, Diarrhoea And Blood In Stool?

Colonoscopy is a very good idea.
Detailed Answer:
Hello!
Thank you for the query.
Lower back pain can be caused by several issues including spine disc problem and urinary tract infection. In your case back pain is not related to blood in stool.
Spine disc problem gives back pain which radiates to leg. It gets worsen after physical activity.
Urinary tract infection gives also lower abdominal pain, frequent urinating, burning while urinating.
If blood in stool is on stool surface or toilet paper, hemorrhoids are most likely the reason. However colon cancer has to be ruled out. That is why colonoscopy is a very good idea and should be done at first. You should also have urine analysis and blood work.
Stool tests should be done if colonoscopy wont find any problems.
For spine analysis, MRI should be done.
Hope this will help. Feel free to ask further questions.
Regards.

Colonoscopy is your first step.
Detailed Answer:
Ca 15-3 is characteristic for breast cancer. It has nothing to do with bleeding from rectum.
Are there any microcalcifications in mammogram?
If there is a lump in mammogram ultrasound should be done.
Pain in rectum along with bleeding are not typical for a cancer. Cancer usually does not give any pain. Polyps also do not give such bleeding along with pain.
So this is rather some hemmorhoids problem or inflammatory bowels disease.
Colonoscopy is what should be done at first.
Regards.
