What Causes Lung Nodules In A Person With Rheumatoid Arthritis?

Question: A chest xray was done in female aged 65 with rheumatoid arthritis. Dr says seen fibroids in lungs due to arthritis .what is tjis exactly?

Well, they are called as lung nodules. it can be seen in patients with rheumatoid arthritis who have had a long standing disease, uncontrolled and with rheumatoid factor positive and can increase at times in patients taking methotrexate.

1. I would like to know if you have any symptoms of cough/shortness of breath due to it or was it an incidental finding?

2. any other site you see the modules?

3. currently what medications and dosage you are taking?

4. duration of your rheumatoid arthritis?

5. how are your joint symptoms?

Joint symptoms very very bad, alot of
Pain but no chest, breathing issues at all.

Arthritis has been present many years but cery bad in last 2,3 years

Only been takin steroids and anti inflammatory medication .

Well you probably have an uncontrolled disease and now you are experiencing extra articular manifestations of it..that is..it is affecting the organs outside the joints.

I am not sure why are they havnt started you on drugs for rheumatoid. Steroids is not a solution for long term and in your age it is gonna cause lot of side effects including osteoporosis, cataracts.

Secondly rheumatoid nodules in the lungs calls for starting treatment.

You can discuss with your rheumatologist regarding methotrxate and leflunomide. Probably combination therapy would suit you better

Well, no they are not a serious thing but more of an indication to start treatment. If they continue to multiply, there is a chance it can involve the wind pipe ( trachea) as well and that time it can cause cough/shortness of breath. Second complication is usually due to infection. Incase it happens in those nodules

All in all, we need to treat them

So are these lung nodules called fibrosis ?

No they arent similar. Infact quite different from each other. Nodules are like small swellings which can be found in other parts of body too. Fibrosis is scarring of the lung tissue. Both can be a part of Rheumatoid ARthritis though and both have a good prognosis when treated.

Ok i see this seems to b fibrosis. Fibrosis is quite hard to treat right n progresses over time?

Well, it is a bit hard to treat no doubt but there are many variants of it and the treatment and progression depends on what type we are treating.

Without a CT of the chest it is very hard to determine what exactly is the type of fibrosis and how it will behave and how strong of the treatment we need.

Secondly get a pulmonary function tests, which will tell us about your lung capacity and how much is the fibrosis affecting your lungs.

Well, it s a bit of a long question to answer here. The most common ones concerning you would be:

1. Non specific interstial Pneumonia--> Associated with Rheumatoid, good prognosis
2. Usual Interstial pneumonia--> Associated with Rheumatoid
3. Idiopathic Pulmonary Fibrosis--> Age related, poor prognosis

Clear de lineation is only on CT Chest as it gives the exact pattern.
