I am Dr Asmeet Kaur Sawhney, a dermatologist.
I have gone through your complaints as well as reviewed the attached photographs.
It seems to be macular amyloidosis which is a chronic disease associated with deposition of amyloid, a protein in the skin.
The exact cause of this deposition is not known but it can occur due to genetic factors and chronic friction occuring as a result of rubbing the skin vigorously with towel or bath sponge .
It is very resistant to treatment.
Various options that can be tried are potent topical steroids, calcipotriol and photo chemotherapy.
For itching you can take antihistaminics like tablet Cetrizine 10 mg once or twice daily depending on the intensity of itching.

Detailed Answer:
Macular amyloidosis is a clinical diagnosis.
It is charactersised by pigmented patches which coalesce to form diffuse hyperpigmented areas.
It is associated with mild to moderate itching and the back and chest are the most common sites.
For 100 percent confirmation you can go for skin biopsy.
In chronic dermatitis there is mostly pigmented papules, nodules and thickening of skin with extensive itching.
It doesn't show the rippled pattern of pigmentation as in macular amyloidosis.
In majority of the patients it is not related to systemic amyloidosis.
As such it is a benign condition not associated with systemic involvement.
It can progress in some individuals to involve chest and upper arms and forearms.
Prognosis is good, the only thing is this hyper pigmentation is resistant to treatment.
