What Causes Mild Abdominal Pain And Stomach Rumbles?

Question: Dear Sir,
I am suffering from mild abdominal pain since last 15-20 days.
This pain comes and goes, usually it appears that pain starts after passing the stools in the morning and goes of as the day progresses. Also usually I don't find any problem when I pass stools in the morning it is normal and little bit semi solid and some times it is semi solid, but, there after I start feeling mild pain in my upper abdomen. also after passing morning stools, most of the time feel that I am about to pass stools but it won't happen (may be for 2-3 times) & I pass some stool with difficulty and that is some times mucus in nature (Not Always). During the start I was feeling full with gurgling sounds in stomach. now this has been reduced and inproved.

I have consulted the physician and he has dignosed that there is internal inflamation due to having some out side food. he has started treatment with some antiboitics like Lcin-oz ( Levofloxacin -250mg & Ornidazole-500mg), Vabact and antacid like Rabiprazole. for 5 days.

I also got done my USG where in it was found everything normal and was informed that it can be gastritis.

My doctor advised me to continue above medicines for 3-5 days more. but now also there is some inprovement like reduction in feeling heaviness.

Please let me know what is this. even after taking anti boitics and antacids symptoms like mild upper abdominal pain which comes in the morning and gets reduced in the afternoon. but somes times in comes agian in the afternoon also and gets reduced.

Please let me know what is this? please guide.

Hi.
Thanks for your query and an elucidate history.

To recapitulate: pain in abdomen 15- 20 days- under treatment of a Physician and being treated for intestinal infection- mild pain starts after passing motion in the morning - feeling of need to pass stool afterwards also - mucus sometimes - initial gurgling settling - USG - medicines continues but no complete relief -
Wants to know what is this and why there is still mild pain in upper abdomen which comes in the morning and gets reduced in the afternoon - comes again in the afternoon sometimes.

I would like to tell in the first place that USG has not much of a role in such problems.
Secondly the pain which comes in the morning after passing stools in the morning and subsides by afternoon and may come back in the afternoon, with mucus sometimes is suggestive of an inflammation of the large intestine (Colitis)

Since you are not getting complete relief with the best ones you are already taking, we have to think differently::
-Change over to Tablets Metronidazole and other antibiotic.
-Undergo colonoscopy and biopsy.
-Get the tests done: Blood -CBC, sugar. kidney and liver functions, Widal for typhoid
-Urine and stool tests

Further treatment can be as per the findings and may Need CT scan of the abdomen if there is no complete relief.

Continue Probiotic, antacid and Rabeprazole; add on activated charcoal tablets and multivitamins.
Also can take anti-spasmodic like Dicyclomin round the clock- meaning at 8 hourly interval.

So continue the treatment and let us see the results of the medicines and the reports of the tests..

Thanks for reply, further I would like to add following information.

My HL AB 27 blood test is positive
I am a ankylosing spondylitis patient.
My blood sugar levels are normal.
However as suggested by you I will go for CBC test.
My intensity of abdominal pain is not increasing. Also some times ( two days back) l feel completely relieved while taking the above suggested medicines.
Some symptoms like feeling heaviness in the stomach has been improved.
I don't find any bloody or black stools. Even stools passed in the morning are very much normal with semi solid in nature. I am not loosing any weight.

Also would like to add that due my spondilitys I am often forced to take anti inflammatory tablets. In recent past and earlier also I have been treated by ortho doctor with pain killers.

Also I have a long history of heart burning may be because of pain killers.

My USG report indicates that " probe tenderness is noticed in epigastric region and suggestive of acute gastritis"

Based on the same my physician has also suspected of gastritis but also has not ruled out the possibility of intestinal inflammation.

He has started following medicines for next 5 days

Vabact
Librax
Buscogast
Nexpro RD

Please let me know what is your suggetion.

Since there is an improvement , it is possible that you will be fine with the medicines your Doctor has given you. Please continue the newer prescription, that you have mentioned.

Also noted your HLA B27 status and ankylosing spondilytis... Always take Ranitidine or Pantoprazole whenever you are taking painkiller medicines to avoid gastritis. Take then with food or a cup of milk / curd.
Upper GI Endoscopy and other investigations are better done with.

USG probe tenderness is just suggestive and not a diagnostic finding. Both the Endoscopies only can tell the correct diagnosis.

You are on the right tract at the moment, please continue the medicines and undergo the investigations that you will discuss with your treating Doctor.
