What Causes Mild Chest Discomfort While On Omeprazole?

Question: Hello,
I am male 30 years and 228 pound , I have chest discomfort in the middle of the chest all the time at rest and Feeling a pulse in left side of the breast . And wink in the heart pain in heart area and some times in stomach area but mostly in heart area. i feel light headed .I am concern because some times i feel that i will get a heart attack. I can do sport without pain and i breath normally. The doctor gave me propranolol 10 mg 1 -3 times a day as need, is helped me a lot for the discomfort but i don't want use it daily so i don't get used to it. I already did EKG and blood test 4 times in ER and the test is normal and i did stress test , ECHO , x ray , radiology, and all this test is normal. the doctor want do catheterization , coronary angiogram but i am very concern about it because has some risk and my father refused me to do it. the medicine that i take regularly amaldopine 5mg for blood pressure , Omeprazole 20 mg , and baby aspirin 81 mg. what should i do . Do you think my heart is fine ? And is there any way to treat the chest pain or the pain in the heart without this test ?

Welcome.
This is Dr Sameer, cardiologist.
Your pain does not look like a cardiac pain. Cardiac chest pain is diffused(i.e. non localized), it increase on exertion & exercise and relieved on rest. I don't see such pain in your case. Also your stress test is negative, which is a indirect marker of any blockage in heart & if negative has around 85-90% sensitivity that their is no blockage in heart.
If you still want to be more sure, you need not do a heart catheterization (angiography), which is the gold standard to detect any blockage in heart, you can in place of that go for a CT Coronary Angiography which is like a CT scan of the heart & is also a very good test to detect any blockage in heart.

My blood pressure was high for one month and i didn't have any discomfort in the chest or in the heart. but after i started blood pressure medicine i noticed the chest pain ? Is it possible the chest discomfort from blood pressure medication because the cardiologist at first said from the medicine and he changed it from lisinopril 40 mg to amaldopine 5 mg but i still have the same chest discomfort.

How did you know the discomfort that i have is not cardiac issue because i watched this tube video and mention the person has normal EKG and stress test and he got heart attack. please see it and let me know if i have the same case

Bp medications does not cause chest discomfort & be assured that this discomfort is not because of medications.
You should go for a stress test to know the cardiac nature of this disease. I should also tell you that stress tests have a sensitive of 85-90%. This means that they can not tell 100% whether their is any blockage in heart vessels or not. So yes even if the stress test is negative, one can have a heart attack.
Better tests are CT Coronary Angiography which has sensitivity of around 97-99% (not 100%).
Gold standard test to detect any blockage is Invasive Coronary Angiography which has sensitive of 100%. And if it does not detect any blockage than their is no chance of getting a heart attack.
So you can choose from any of these tests.

I already did stress test and the doctor said is normal. is CT Coronary Angiography the same as catheterization test where they use a thin, flexible tube to injected with a XXXXXXX ?
because the doctor said CAT and i heard the nurse said CT but i am not sure if they mean coronary angiogram test where they put a tube and injected with XXXXXXX or they mean only the one with x ray ?
is coronary angiogram the same as CT Coronary Angiography ?

Welcome back.
CT Angio is different from invasive coronary angiography.
In CT Coronary angio, a dye(medicine) is given from one of your vein & than CT scan of heart is done.
In invasive coronary angiography, we put a catheter in one of your arteries (either leg or arm) & insert a small wire that goes till the heart. We than insert a XXXXXXX through that wire & see the blood vessel of heart. It is like direct seeing.

But do you know which one that the doctor will do because i heard him say CAD or CAT but i am not sure one they will do because if it's CT Coronary angio i will do it but if it's the other one maybe i won't do it as looks more complicated .Is CT Coronary Angiography the same as radiology because i did it before in ER and they said is normal and the doctor told me the pain probably not from my heart ?

Hi,
Welcome back.
I think he wants to do the invasive coronary angiography (CAT- Coronary Arteriograhy). CAD is coronary artery disease.
If you don't want to do it you can ask your doctor for CT Coronary Angio.
But i will recommend you to go for invasive one, because if the CT Angio shows some blockage than to exactly quantify the blockage you have to go for invasive angio. So why not directly go for it.

Also it is not as complicated as it sounds. It just takes 15-20min & will be 100% sure about your blockage. It can be done from the arm & you can be discharged the same day. So this is better for you.
Still if you don't want it, ask your cardiologist that you want to do CT Coronary Angio.

is CT Coronary Angio the same as Radiology . Because i already did radiology in ER and do i need to do CT Coronary Angio ?I called them and they said CT Coronary Angio but i am not sure if it's the same as Radiology

Their is no specific test called Radiology. Radiology is a group of tests which includes X Ray, CT, MRI. So i think you must have got a X Ray done.
You have to enquiry whether CT Angio has been done or not. In CT Angio we put a XXXXXXX through the vein & than do a CT.
If such thing was done than it was CT Angio & if not done you should get it done or the invasive coronary angiography which i told you earlier.

I did x ray than they did radiology where they put me inside the machine and they told me to hold breathing for few second They also injected me with morphine . is this CT Angio?
. they did the radiology two times because first one is wrong. I am concern if i do CT Angio because it has a lot of radiation and i already exposed to the radiation when i did x ray in the chest , stress test, ECHO test, radiology 2 times in the chest and these was all in pas 2 month and worry if i get a cancer from these devices as radiation effect the body cells.

Sorry for delayed reply.
The test in radiology which was done looks to be CT, but we don't give morphine, we give a XXXXXXX which can visualize the blood vessel. So it does not seem to be a CT Angiography.
They should have given a report of the test which was done. You can check the details of the test in that & can send me too.
Further yes these all tests expose you to a lot of radiations & you should avoid doing it very frequently. X Ray expose to a very small amount of radiation but CT exposes to good amount of radiation. Echo does not have any radiation & is absolutely safe. Don't worry is caused by very frequent exposure of large amount of radiation for a long period of time.

So after all this if the symptoms currently are not very much you can postpone all the investigations & just go for a STRESS ECHO (in which we do a echo first, than we ask patient to walk on trade mill to increase the heart rate & when a target heart rate is achieved, we again do a echo to see the effect of stress on heart). This test is absolutely radiation free & as i explained in my previous answer also that this is a very good test to rule out any underlying heart disease & has a sensitivity of around 85-90%. If negative you can relax & plan for a invasive angiography or CT Angio after a year or if you have any new symptoms.
So don't burden your body with lots of investigation & just do this simple & good test.
