Brief Answer:
hyperprolactinemia + PMS

Detailed Answer:
Hello,
Thanks for contacting HCM,
If your nipples are secreting liquid and your pregnancy tests are negative , with irregular periods , it is likely that your prolactin levels are high and that is the probable cause of irregularity in menses. This is known as hyperprolactinemia and needs to be ruled out by getting serum prolactin levels done. If found to be high then A MRI brain would be required to look for any microprolactinomas in the pituitary. If ruled out then you may be given dopamine agonists like bromocryptine to bring the prolactin levels normal and hence the regularity in cycles.
Also back pain , nausea and diarrhoea are part of PMS or premenstrual syndrome which indicate that your menses are to ensue in few days and these are normal , mimic early pregnancy symptoms but subside once the periods start.

Detailed Answer:
if you have had your periods recently then nausea with diarrhoea indicates a gastrointestinal infection possibly and a lower back ache can be seen in n accompanied urinary tract infection . This has to be evaluated with a general physical examination by your physician and with a urine culture respectively.
