What Causes Miscarriage In A Woman With PCOD?

Question:

I am married since 3 and half years. I have irregular period ranging from 30 to 40 days

8-04-2013:
I went through preventive health check up on as per them
Left ovary size was 3.8* 2.2 * 2.5 cms Vol-11.2 cc
Impression: Bilateral Polysystic ovaries

Hormone Test report
Isulin: 6.88
Testerone,Total:31.22
Testerone Free: 0.79
Glucose, serum : 81
FSH: 10.78
LH: 5.96
Prolactin: 20.85

i was asked to take for two months zomet sr and fol 123
Note: My mother is Diabeic and HT

May 2014
My husband had earlier ejaucalation and erection problem, which is rectified now.
His hormon test and ultrasound is normal

8th October 2014:
I had gone to doctor in 8th October 2014 (second day of period), doctor asked me to take thyroid test. She asked me to take
apcod obis, cal fol 123 and Siphene and cabgoline for 5 days

Thyroid T3 0.97 ng/ml, T4 7.19 mcg/dl and TSH was 3.28 mcIU/ml
Thyroid was normal

7th October
We went through follicular study on doctor asked to try on 23rd th and 24th of october my LMP was 7th OCtober

6th November
I saw brown spotting on 6th , on 8th we travelled to native i had severe cramps and pain all night
My period started with brown lumps and heavy pain in pelvis area
From 9th morning it was fine
I have bleeding for 5 to 6 days

As per doctor's advise we had gone for follicular study -- I had not taken any medicine this time

Based on Follicular study it ruptured on 27th November, on that day one 5000 mg Hcg injection was suggested

We were asked to try on 26,27,28

We tried on 29 as well

I missed my period on 8th December (my period cycles were irregular ranging from 30-40 days)

Based on doctor advise tried urine pregnancy test on 15th December which came positive

Same day we went through blood hcg test on 15th it was 539.20 MIU/mL which as per Biological reference is 3 week

20/12/2014
doctor asked to take ultrasound and start taking sustain 300mg
Impression: Bulky uterus showing decidual reaction
Asked to repeat on 3rd December

December 2nd
I saw brown spotting very little

On 3rd the ultrasound Says
There is well defined Intra Uterine Sac
Msd 11MM
Coresponing to gestational age is 5-6 weeks
No foetal pole appreciated
The cervical internal os is closed
No collection seen
Both Adnex are normal
Impression: Early Intra Uterine Gestation- to follow after two weeks


I had heavy bleeding with some lumps on 4th which continued uptill 5th
On 5th doctor asked me to take scan as per scan size of sac had rduced to 0.6 cm which is less than 5 weeks
Sac shows no defiitive yolk sac / fetal pole
The cervical internal os is closed, Cervix measures 3.1 com
No collection seen
Both Adnex are normal
Uterus is anteverted
Uterus cavity shows a well defined sac located at the fundus with good trophoblastic reaction around it

Doctor asked to see for 10 days

I had heavy bleeding with lot of lumps till 5th night and 6th morning within one hour my pad was soaked in blood with big lumps

Doctor suggested i had spontanious miscarriage
Based on doctor advise took "Misoprost 200mg"
I was asked to stop taking all medicines till two months

My question why did i have miscarriage

Do i need to do any tests so that it does not repeat again ?
Do my husband need to do any sperm analysis test?
When can we try again after this?

Hello Mrs XXXXXXX its really sad to read the sequence of events.
Miscarriage in your case is most probably due to bad egg quality which is common in polycystic ovary syndrome.
Your husband's test will not reveal any information.
From next cycle onwards you should take HCG injection and progesterone injection to support pregnancy from the very start and also take full bed rest.
Take a rest of at least 3 months before next induction of ovulation

Hi Doctor,

This time also I was on Sustain 300mg which is for progesterone and was given HCG injection then why did it happen?

Also is there any test which I can take to see my egg quality.

Also this time nothing was induced for ovulation, I ovulated on my own.

Is there any remedy or anything else which can help PCOD to go away completely.

Detailed Answer:
PCOD is a difficult to treat problem but you can try certain supplements like myo-inositol and pyridoxine. There is a preparation called fertisure - F, it is very good and if taken for 3 months improves egg quality a lot.
There is no test to see egg quality per se but in ultrasound for follicular growth the regularity of the egg and number of eggs gives an indirect evidence of quality of the egg.
Regular medicine intake and rest is the cornerstone of treatment for you in your next pregnancy.
